CHAPTER ONE  INTRODUCTION

1.1  Identity construction in digital media 

In recent years, clinical discourse has garnered substantial attention as a hotspot in discourse studies. The reason behind this trend lies in the potential benefits this area of research could provide to the medical field. The unique nature of language interaction within clinical settings offers rich linguistic data for researchers, and can provide more scientific and systematic guidance for improving doctor-patient communication, thus playing a positive role in enhancing the quality of medical services. Within the realm of clinical discourse research, the construction of medical staff’s identities is an issue worthy of attention. Medical staff play pivotal roles in clinical communication, with their identity construction not only impacting the efficacy of doctor-patient communication, but also significantly influencing the collaborative efforts within medical teams and the treatment outcomes for patients. Therefore, a profound study of the construction of medical staff’s identities contributes to understanding their role positioning and responsibilities within clinical environments.

CHAPTER THREE  THEORETICAL FOUNDATIONS

3.1  Fairclough’s approach of critical discourse analysis

Critical Discourse Analysis (CDA) is widely recognized as a paramount tool for discourse analysis within critical language research. Its primary objective is to uncover hidden ideologies and power dynamics within discourse, shedding light on how discourse influences mainstream ideologies. Fairclough views discourse or language usage not merely as individual actions but as social practices, signifying that discourse serves as both a mode of action and representation (Fairclough, 1992). There exists a dialectical relationship between discourse and social practices: discourse is fashioned within specific social contexts while concurrently shaping those contexts (Fairclough, 1992). Fairclough emphasizes that discursive practices mediate between texts and social practices, and these practices are intertwined with the process of production, distribution, and consumption, all of which are socially bounded actions that necessitate examination within particular economic and political frameworks.

In pursuit of a more comprehensive discourse analysis model that integrates text-oriented and social-oriented approaches, Fairclough introduced a three-dimensional conceptual model of discourse. This model offers a detailed analysis of the nuances of discourse across various levels, making it well-suited for examining the construction of meaning in multimodal documentaries and unveiling the underlying ideologies and power dynamics within these modes of expression. 

CHAPTER FIVE  MULTIPLE IDENTITIES OF MEDICAL STAFF AND MULTIMODAL CONSTRUCTION

5.1  Professional medical staff

In medical documentaries, medical staff hold a specific institutional identity conferred by healthcare institutions. In this context, medical staff are regarded as authorities within the healthcare establishment, possessing professional and authoritative medical scientific knowledge relative to, filmmakers, and audiences. Medical staff engage in various activities including medical practices, consultations, discussions with colleagues and medical practices. These situations encompass a wide range of professional knowledge and practical skills. As a multimodal discourse, the documentary effectively constructs the professional identity of medical staff through diverse modalities.

5.1.1  Participants in consultations and discussions 

Covid-19 was first identified at the end of 2019. Therefore, as among the first group involved in treating patients with the novel coronavirus, medical staff featured in the selected documentary of this thesis often engage in impromptu or organized discussions and consultations regarding the patients’ conditions. By analyzing the data, the author discovered that medical staff encountered difficulties multiple times and often engaged in discussions with colleagues.

5.2  Ordinary personnel

The documentary constructs medical staff’s identity as ordinary individuals with semiotic resources when the audience sees medical staff facing life’s challenges, family relationships, and their responsibilities to society and their profession. In this way, the audiences are more likely to develop empathy and resonance since emotional resonance can deepen the audiences’ understanding and support for medical staff. This section will study the multimodal construction of ordinary personnel identity in the discourse of the documentary. 

5.2.1  Multimodal construction of ordinary personnel identity

In visual grammar, attitude encompasses the choice of vantage points, with two distinct perspectives shaping the portrayal of participants: one from a vertical angle, and the other from a horizontal angle. Consequently, visual representations can be interpreted in such a way as to prompt an exploration of whether to embrace a subjective or objective standpoint. Subjective images are those in which the composition encourages viewers to adopt a specific position within the image, often leaning towards naturalism rather than symbolism. Horizontal angles establish an impartial and objective relationship between the participants and the viewers, while vertical angles accentuate power dynamics between the viewer and the image. High angles endow viewers with authority, whereas low angles bestow the portrayed participants with a sense of eminence and authority surpassing that of the viewers (Kress & van Leeuwen, 2021).

CONCLUSION

Major Findings and Implications of the Study

Drawing on Fairclough’s three-dimensional framework, this thesis embarked on a three-phase analysis of documentary discourse that aligned with three research questions. 

 In order to address the first two research questions, the study conducted a multimodal critical discourse analysis of the documentary discourse, The Red Zone, analyzing the identities constructed for medical staff and the means by which these identities are constructed. The findings are as follows: 

Firstly, regarding the identities constructed in The Red Zone, the study revealed that the multimodal discourse constructs four identities for medical staff, including their professional identities as doctors or nurses, their identities as ordinary personnel, as friends of patients, and as organizers. Concerning the construction of these multiple identities, following a multimodal critical discourse analysis based on systemic functional linguistics and visual grammar, the study found that medical staff’s identities are collaboratively constructed through a variety of modalities, including visual modalities, verbal modalities, and auditory modalities. The documentary’s examination of both visual and verbal modalities, along with their intrinsic interconnections, illuminated the multiple identities ascribed to medical staff. This study found that the discourse of medical documentaries has an institutional nature. Medical staff, depicted within healthcare establishments, are naturally endowed with a professional identity reflective of their institutional roles. To be specific, the professional identity of medical workers is constructed through presenting them as participants of discussions and consultations and medical practitioners. Beyond the bounds of professional duty, especially in the milieu of an epidemic’s nascent stages and the working conditions, the documentary casts medical staff in three additional, deviational role identities: as ordinary personnel, showcasing their real lifestyle; as friends, humanizing the patient experience; and as organizers, embodying structured leadership and crisis management.
